120 Biomarkers and What They Mean: A Woman’s Guide to Reading Her Own Labs
Has a doctor ever handed you lab results and said, “Everything looks normal,” before you even had a chance to ask a question?
Maybe you nodded and left. Maybe you went home and Googled everything on the printout and ended up more confused than when you started. Maybe you accepted “normal” because you didn’t know enough to push back, even though something in you felt like the answer wasn’t complete.
You’re not alone. And you’re not wrong to want more.
This women’s lab work guide is going to give you enough context to understand what your labs are measuring and why it matters. Not enough to be your own doctor. Enough to walk into your next appointment knowing what questions to ask.
Why Standard Labs Often Miss the Point
Before we talk about what’s in a comprehensive panel, it helps to understand why the standard panel your primary care doctor orders often falls short.
Standard blood panels are designed to catch disease. They’re looking for values that are dramatically outside of normal — signs of anemia, kidney failure, diabetes, thyroid disease. They’re a safety net, not a wellness tool.
Comprehensive functional labs are different. They’re looking at how well your body is functioning, not just whether something is clinically wrong. The difference between those two things is enormous. You can have labs that show nothing alarming and still be running on depleted reserves, with hormones that are technically in range but far from optimal, with inflammation quietly building, with a thyroid that is struggling but not yet failing.
That’s the gap. That’s why so many women are told they’re fine when they don’t feel fine. And as we explore in Why Your Doctor Says You’re Fine (And Why You Know Something Is Wrong), the gap isn’t a coincidence. It’s a feature of a system designed to catch disease, not optimize health.
What 120 Biomarkers Are Actually Looking At
A comprehensive panel covers several major categories. Here’s a plain-language overview of what each one tells us.
Metabolic health. Blood sugar, A1C, kidney and liver function markers. This is how well your body is processing and clearing what it takes in. When these are off, everything downstream is affected.
Blood and oxygen. Red blood cells, white blood cells, hemoglobin, and related markers. This is where we look at ferritin, the stored form of iron, not just the iron in circulation. That distinction matters enormously and is almost never included in a standard panel. As we covered in The Postpartum Nutrient Crash Nobody Warned You About, ferritin falls first and falls furthest, but hemoglobin can look completely normal for months.
Inflammation. C-reactive protein and related markers tell us whether your body is in a state of chronic low-grade inflammation. This often exists well below the level where it would trigger a diagnosis, while still significantly affecting how you feel.
Thyroid function. A full thyroid picture includes TSH, but also free T3, free T4, and thyroid antibodies. TSH alone is not enough. Free T3 is the active hormone your cells actually use. TSH tells you what your brain is asking the thyroid to do. Free T3 tells you whether the thyroid is actually doing it. Very different questions.
Hormones. Estradiol, progesterone, testosterone, DHEA, LH, FSH, prolactin, and SHBG together tell us the full story of what’s happening across your reproductive and adrenal hormone system. One or two of these markers in isolation is like reading every other page of a book. You get some of the story. Not all of it.
Nutrients. Vitamin D3, B12, magnesium, iron, and others. These are the raw materials your body uses to produce energy, regulate mood, support immunity, and keep hormones in balance. When they’re low, everything suffers. And they’re rarely included in a standard panel.
Cholesterol and cardiovascular markers. Beyond total cholesterol, a comprehensive panel looks at HDL, LDL, triglycerides, and inflammatory cardiovascular markers. Kinda like the difference between checking whether a car has gas versus checking whether the engine, brakes, and oil are also working.
What “Normal” Actually Means
Here’s something I want every woman to understand before she ever looks at a lab result again.
Reference ranges are not the same as optimal ranges.
A reference range is the range within which most people in a large population fall. It doesn’t mean everyone in that range is healthy. It doesn’t mean being at the very bottom of the range feels the same as being in the middle of it. And for hormones especially, the range is wide enough that a woman can be at the low end, symptomatic, and struggling, and still be told she’s normal.
Optimal ranges are narrower. They’re the values associated with feeling well, functioning well, and having the reserves your body needs to handle the demands of your life. That’s a different conversation than the one most lab reports are having with you.
When I review a client’s labs, I’m not just checking whether each value falls within the reference range. I’m looking at where it falls, how it relates to her symptoms, and how the whole picture fits together. Because a thyroid at the bottom of normal, combined with iron at the bottom of normal, combined with vitamin D at the bottom of normal, is not a picture of a woman who is fine. It’s a picture of a woman who is barely managing and doesn’t know why.
That woman deserves more than a “normal”.
What Hair Tissue Mineral Analysis Adds
Blood work tells you what was circulating on the morning your blood was drawn. Hair tissue mineral analysis looks at three to four months of mineral status. It shows me patterns of depletion, toxic metal accumulation, and adrenal stress over time that a single blood draw never captures.
Think of it as a blood draw being like a photograph; a single moment in time. Hair tissue mineral analysis is more like a video. It shows you what’s been happening over months. For many clients, that’s where the most significant findings live.
